Local Exhibition Game Request Form

Host team (host being whose ice it's on or who has booked the ice) will complete this application in advance to inform WMHA and Hockey Yukon about your intention to host an exhibition game in Whitehorse. Please try to complete a minimum of 7 days in advance. Officials will automatically be booked on receipt of this form. 

Prior to submitting this form ensure you are prepared to input BOTH rosters (home and away) and include the names of AP players if they are being utilized. 

Game sheets must be kept by home team manager and submitted to office@whitehorseminorhockey.ca 

Host team will be charged for the officials and the ice.
